This page has two parts. Part 1 is our website privacy policy, which explains how the diablochiro.com website handles information. Part 2 is our Notice of Privacy Practices, which explains how the practice handles your protected health information under HIPAA. Dr. Salem prefers to keep them together, so you have everything in one place.
Part 1: Website Privacy Policy
Diablo Chiropractic (“we,” “us,” or “the practice”) operates the website at diablochiro.com. We respect your privacy and keep our data practices simple. This policy describes what the website does and does not do.
Information we collect
- Information you choose to send us. Our website does not have contact or intake forms. If you reach out by phone, text, or email using the details on our Contact page, we receive whatever you include in that message (for example your name, phone number, email, and the reason for your inquiry). We use it only to respond and to help schedule your care.
- Information collected automatically. Like most websites, our web server records standard technical information such as your IP address, browser type, and the pages you view. If we enable a website analytics tool in the future (such as Google Analytics), it would collect similar usage information through cookies to help us understand how the site is used.
Cookies and similar technologies
- Preference cookie. Our site sets one small first-party cookie (named
lang) that remembers a language preference. It contains no personal information. - Google Fonts. Our pages load fonts hosted by Google, so your browser contacts Google to retrieve them, which involves sharing your IP address with Google.
- Google Maps. Our Contact page embeds a Google Map. When that map loads, Google may receive your IP address and set its own cookies, subject to Google’s privacy policy.
- Analytics (if enabled). If we add an analytics tool, it would use cookies as described above.
- Embedded video (Vimeo). Some pages embed a video hosted by Vimeo. When the player loads, Vimeo may receive your IP address and set its own cookies, subject to Vimeo’s privacy policy.
You can control or delete cookies through your browser settings, and you can set most browsers to refuse cookies. Some features, such as the embedded map, may not work as well if you do.
Links to other websites
Our website links to third-party services we recommend, including our Fullscript and Metagenics supplement dispensaries and our Saatva mattress referral link. When you click one of these links you leave our site, and the other company’s privacy policy governs your visit there.
Affiliate disclosure. Some of these are referral or affiliate relationships, which means the practice may receive a small benefit if you make a purchase. This never changes our clinical judgment; Dr. Salem recommends these products because she believes in them.
How we use information
We use the information described above to respond to your inquiries, to schedule and provide care, to operate and improve the website, and to comply with the law.
How we share information
We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We share information only with service providers who help us operate (for example our website host), or when required by law or to protect our rights.
Your California privacy rights
If you are a California resident, the California Consumer Privacy Act (as amended) gives you the right to know what personal information we collect, to request that we delete or correct it, and to opt out of the sale or sharing of your personal information. As noted above, we do not sell or share your personal information. To exercise any of these rights, contact us using the details below.
Children’s privacy
Our website is not directed to children under 13, and we do not knowingly collect personal information from them through the website.
Changes to this policy
We may update this policy from time to time. When we do, we will revise the “last updated” date above.
Part 2: Notice of Privacy Practices (HIPAA)
This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Our commitment
Diablo Chiropractic is required by law to protect the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information
- Treatment. We use your health information to provide and coordinate your chiropractic care, and we may share it with other providers involved in your care.
- Payment. We may use and disclose your health information to bill and obtain payment for the care we provide.
- Health care operations. We may use your health information to run our practice, such as quality review, staff training, and scheduling.
- As required or permitted by law. We may disclose your health information when required by law, for public health activities, or to prevent a serious threat to health or safety.
Other uses and disclosures, including most uses for marketing and any sale of your health information, will be made only with your written authorization, which you may revoke at any time.
Substance use disorder treatment records
To the extent that we have substance use disorder patient records about you that are subject to federal confidentiality rules (42 CFR Part 2), we will not use or share information in those records in civil, criminal, administrative, or legislative investigations or proceedings against you without (1) your written consent or (2) a court order and a subpoena. If we ever contact you for fundraising using Part 2 information, we will give you clear and obvious notice in advance and a choice about whether to receive those communications.
Your rights
- To inspect and receive a copy of your health information.
- To request a correction to your health information.
- To request confidential communications (for example, that we contact you only by a certain method or at a certain location).
- To request a restriction on certain uses and disclosures.
- To receive an accounting of certain disclosures we have made.
- To receive a paper copy of this notice on request.
- To file a complaint if you believe your privacy rights have been violated, with us or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
Our duties
We are required to maintain the privacy of your health information and to notify affected individuals following a breach of unsecured health information. We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as any information we receive in the future.
